The State of Nevada Purchasing Division, on behalf of multiple state agencies, is seeking Statements of Qualifications from qualified providers to deliver direct client services in various areas.

MEDICAL AND RELATED SERVICES: Providers are to reference the Scope of Services Attachment for common descriptions of services related to the Medical services. Provider will then ensure "see quote attachment" is checked for this item if their services is included in the medical services description. Provider shall complete the Direct Client Services Submission Packet and use the included Service Descriptions Table to indicate the types of services they provide.

BEHAVIOR, COMMUNITY BASED AND RELATED SERVICER: Providers are to reference the Scope of Services Attachment for common descriptions of services related to the Behavior and Community Based services. Provider will then ensure "see quote attachment" is checked for this item if their services is included in the services description. Provider shall complete the Direct Client Services Submission Packet and use the included Service Descriptions Table to indicate the types of services they provide.
